I HEREBY ASSUME ALL OF THE RISKS ASSOCIATED WITH AXE THROWING, including by way of example and limitation, any risks that may arise from negligence or carelessness on the part of Savage Axe, LLC, its members, managers, employees, and agents, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

I acknowledge that axe throwing may involve a test of a person’s physical and mental limits and carries with it the potential for death, serious injury, and property loss. These risks are not only inherent to participants, but are also present for bystanders and other patrons on the premises.
I certify that I am physically fit, have sufficiently prepared or trained for participation in axe throwing, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems that prelude my participation in this activity.

I WAIVE, RELEASE, AND DISCHARGE Savage Axe, LLC, its members, managers, employees, and agents, from any and all liability arising out of axe throwing on the premises. A hereby waive any and all rights, claims or actions which may arise out of any my death, disability, personal injury, property damage, or damages of any kind, including but not limited to any claims arising out of the fault of Savage Axe, LLC, its members, managers, employees, and agents.
I agree to INDEMNIFY, DEFEND, AND HOLD HARMLESS Savage Axe, LLC, its members, managers, employees, and agents from any claim, demand, or action which may arise out of any my death, disability, personal injury, property damage, or other damages of any kind related to axe throwing on the premises.

I hereby consent to receive medical treatment that may be deemed advisable in the event of injury, accident, and/or illness during this activity.

I understand that while participating in axe throwing on the premises, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by Savage Axe, LLC.

The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
